Here is some basic information about how we work.

First Appointment

New patient appointments are typically 1 to 1.5 hours. Dr. Bazzan really wants to know you, your health history, and goals to create a targeted, customized evaluation and treatment plan.

Follow up Appointments

Follow up appointments range from 30 minutes to 1.5 hours depending upon the individual’s needs. All test results will be reviewed during your follow-up appointments.

Open Phone Hour

Dr. Bazzan is available for brief conversations during the Open Phone Hour Monday, Wednesday, and Thursday from 8 am to 8:45 am EST.

Questionnaires

Before your first appointment you will need to complete and return our detailed medical questionnaire. This valuable information will be reviewed by Dr. Bazzan prior to your consultation.

Payment

FWSI does not participate with any health insurance companies. We are a fee-for-service office and payment is due at the time of service. We accept Visa, MasterCard, checks, and cash.

Appointment Time

We recognize that managing time appropriately takes an effort on everyone’s part, and we do everything possible to see patients at their scheduled appointment time.

Cancellations

As we have a very busy practice, we highly value our time management.  Our goal is to provide each and every patient with excellent medical care in a timely fashion. Everyone’s time is valuable! FWSI provides all patients with our guidelines for cancellations, no-shows, and lateness.
